***INSURANCE COMPANIES WILL TYPICALLY ONLY PROVIDE PAYMENT FOR TREATMENT OF MAJOR MENTAL DISORDERS SUCH AS DEPRESSION, ANXIETY, BIPOLAR DISORDER, TRAUMA, ETC. INSURANCE WILL TYPICALLY NOT REIMBURSE FOR OTHER COUNSELING NEEDS – SUCH AS MARRIAGE COUNSELING, FAMILY COUNSELING, PARENTING HELP, PORNOGRAPHY ADDICTIONS, ETC. OUR OTHER PAYMENT OPTIONS SUCH AS DISCOUNTED RATES AND NO-INTEREST FINANCING CAN HELP MAKE COUNSELING MORE AFFORDABLE WITHOUT DEALING WITH THE LIMITED COVERAGE OFFERED BY INSURANCE PLANS. How do I use my insurance?

  1. Does my plan cover counseling for what I need help with?
    • Before coming in for counseling you need to make sure that your insurance will cover the issue for which you are seeking help***. Contact your insurance company and/or plan documents for information.
  2. Do I need a referral to use my insurance?
    • Health Maintenance Organization Plans (HMO): You will typically need to get a referral from your assigned primary care physician BEFORE attending counseling if you want to use your insurance.
    • Preferred Provider Organization Plans (PPO): No referral is typically necessary to use your insurance.
    • Exclusive Provider Organization Plans (EPO): No referral is typically necessary to use your insurance.
  3. What does Swinton & Associates Counseling need from me BEFORE I schedule an appointment?
    • Provide a picture or scan of the front and back of your insurance card. Email this to our billing manager at email hidden; JavaScript is required so she can confirm if you need any pre-authorizations or if there are other requirements before you are seen by a counselor. She will respond within 1 business day.
  4. What do I need to do AFTER I schedule an appointment?
  5. What will it cost me at my appointment?
    • Copay Plans: You will be responsible to pay your copay at the time of service
    • Non-Copay Plans: You will be responsible to pay $60 at the time of service. The difference will either be charged or credited to you once your insurance company has processed the insurance claim.

Out-of-Network Coverage

For or diagnoses that are covered by insurance***, we can provide you with detailed superbills at the time of service for you to submit to your insurance company directly to seek out-of-network reimbursement (contact your insurance company to inquire if you have out-of-network coverage). You pay at the time of service and then submit the form we provide to your insurance company to seek reimbursement directly. This is available with all of our counselors.
